Pain during ovulation: mid-cycle pain

Around 20% of women experience pain during ovulation – known as mittelschmerz. The symptom manifests itself in many different ways: from a diffuse ache in the lower abdomen to sudden sharp pains. The duration varies from a few minutes to several days, and the pain may be felt on one side or in different parts of the body.
What is Mittelschmerz?
Mittelschmerz is pain in the middle of the cycle that occurs around the time of ovulation. It is named after its timing in the cycle – in the middle of the menstrual cycle.
Possible causes
The exact cause has not yet been fully established scientifically. The following factors are under discussion:
- Capsular tension caused by the growing follicle
- Irritation of the peritoneum caused by fluid when the follicle bursts
- Contraction of the fallopian tubes following ovulation
What helps with mid-cycle pain?
The following measures may provide relief:
- Heat – a hot water bottle or cherry pit pillow placed on the lower abdomen
- Magnesium – can help reduce cramps
- Gentle exercise – light walks
- Massages – gentle abdominal massages
Is mid-cycle pain a reliable sign of fertility?
No. Mid-cycle pain is not a reliable sign of ovulation because:
- It does not occur regularly and does not affect all women
- It sometimes only begins after ovulation
- It can also occur without ovulation
Cervical mucus is considered a much more reliable sign of the fertile phase.
When should you see a doctor?
Persistent, severe groin pain requires medical assessment to rule out more serious conditions (e.g. cysts, endometriosis).
